Housing Stabilization Services

Housing Stabilization Services (HSS) is a new benefit for individuals on Medical Assistance to help find and maintain housing.

 

Housing Stabilization Services has three parts:

Housing Consultation

Helps eligible individuals without a case manager or targeted case manager create a person-centered plan and help submit the plan to a Housing Transition provider.

Housing Transition

Helps eligible individuals find a home of their own.

Housing Sustaining

To help maintain housing and prevent homelessness.

Who is Eligible?

A member is eligible for housing stabilization services if they meet all of the following needs-based criteria:

Be on Medical Assistance (MA)

Be 18 years old or older

Proof of disability or disabling condition.

Proof of housing instability.

Proof of assessed need for services
